We do not use the Services to collect, store, or process any protected health information ("PHI") subject to the Health Insurance Portability and Accountability Act (“HIPAA”), any applicable health privacy regulation or any other applicable law governing the processing, use, or disclosure of protected health information. When you fill out your PHI, and that form is connecting to the secure private web service on Amazon AWS / Google Cloud which is currently HIPAA-certified.
Who needs to be applied/verified?
Prescription Verification only applies to US and South Korean consumers. MocoQueen deeply cares about your health. Under the Fairness to Contact Lens Consumer Act (FCLCA), we are required to verify your contact lens prescription with your order.
How would you provide your prescription?
Option 1. Send us a copy of your prescription
Please submit a copy of your valid prescription issued by an eye care professional. You can either scan or take a photo of it. Kindly include your name and upload the image of your prescription through the provided Google form.
The verification process takes 8 business hours and if no response received from your eye doctor, your contact lens prescription will be assumed as valid and your order will be processed and shipped.
Option 2. Submit contact info of your eye care professional / optometrist / optician.
Kindly provide us with the necessary information, name and phone number of your eye care professional.We will contact your eye care professional and verify the accuracy of the prescription. Your eye care professional has eight business hours to respond (extended to the immediate working day upon weekends or public holidays). If your eye care professional does not respond within the required time, the prescription is verified automatically, and we may process and ship your order.
